Here is the information from the matrix above for Standard IV in a word document formatted to print on business cards (Avery 5877) for use in PD. The format has the evidences/artifacts for the front of the cards on the odd pages. You then can print the back of the card with the subsequent even page for the standard/element and corresponding performance level. Please learn from my trials and error...be sure to use business cards that are designed for 2-sided printing and are either perforated or clean-edge (which keeps you from having to cut anything).
